Abstract: Very often, in the treatment of complex aortic aneurysms, the vascular surgeon is faced with a huge challenge when it comes to preserving the internal iliac artery in patients. A number of techniques have been described in the literature, such as embolization, snorkel, chimney, etc. Over time, in the last few years, there has been talk of an iliac stent graft with a surgeon-modified branch to the internal iliac artery. In this way, blood flow through the internal iliac artery to the organs in the pelvis is preserved, and this method is also used in type Ib endoleak, when it comes to a complex aortic aneurysm. In this way, type Ib endoleak is corrected in one step, and the patency of the internal iliac artery is also preserved.
Key words: complex aortic aneurysms, internal iliac artery, iliac stent graft
