Abstract. The management of malignant melanoma continues to predominantly involve surgical intervention. A crucial aspect of the therapeutic strategy is the sentinel lymph node biopsy, particularly valuable for detecting micrometastases in regional lymph nodes. Cases of secondary lymph node drainage to atypical lymphatic collectors have been documented. In this case study, we outline our clinical methodology for a patient with histologically verified malignant melanoma. Utilizing skin lymphoscintigraphy, we map out the peritrochanteric and axillary lymph nodes after local excision of the lesion. This innovative surgical approach allows us to anticipate the course of the disease and minimize the risk of postoperative complications.
