Abstract. Echinococcus granulosus remains a widespread disease in some endemic regions of the world. Candids and ugulates are the definitive and respectively intermediate hosts and the human acts as an aberrant host for this parasite. Symptoms of hydatidosis mostly depend on the cyst’s size, location, type and whether it is complicated or not. Diagnosis is certain when using serology and the ELISA method, however, in recent years imaging studies with ultrasound at the forefront, gain a wider use in determining the presence and state of any cystic lesion. Even after years, surgery remains the main treatment option for large and complicated hydatid cysts. In the reported clinical case we present a patient with a secondary disseminated echinococcus cyst, extending from the upper abdomen to the left side of the chest. Our goal is to highlight the difficult anatomy and potential com¬plications that might occur and how to avoid them.

Key words: hydatid disease, thoraco-abdominal involvement, disseminated echinococcosis
