Abstract. Managing patient flow in outpatient care is a key factor in the efficiency of the healthcare system. The article explores the main challenges in coordinating between primary care and hospital treatment, as well as the best practices and models for optimization. It highlights integrated health networks, multidisciplinary teams, telemedicine, and preventive programs. Guidance is provided to general practitioners for better patient flow management. The article aims to support the development of sustainable solutions to reduce unnecessary hospitalizations and improve health outcomes.
